Community Outreach Hubs

Tackling loneliness and isolation and improving older people’s independence is a high priority for our organisation. We know that recent changes and challenges within Neath Port Talbot that has seen further closures of local shops, post offices, chapels, churches, local day services and a reduction in public transport directly influence an older persons independence and quality of life. 

We have several Community Outreach Hubs located across the County Borough, and are always looking for new accessible locations. These act as an extension to our current services and ensure that local people have direct access to our services.  Currently we our weekly Outreach Hubs take place at the following venues:
  • Neath Town Hall - Monday's 10:00 - 12:30
  • Bertha Road Community Centre, Margam - Tuesday's  10:30 - 12:30
  • Pontardawe Library - Thursday's 13:00 - 15:00
The aim of the Outreach Services is to provide a holistic intervention programme to increase prevention and provide early intervention of physical and emotional support. Activities are fun and user led as we aim to create a positive experience of ageing, with local residents feeling involved, appreciated, more confident and active within their community. Last year, we delivered 600 Community Outreach activity sessions.
 
Our Outreach Hubs can help you live life to the full by providing a wide range of self-sustaining activities that encourage and improve physical and emotional health and well-being. We also work in partnership with a wide range of providers and local groups who offer community support and are confident that there is something for everyone to enjoy. Activities are delivered through various methods and services including the following:
  • Information and advice
    This service covers topics including welfare benefits, health, housing, free legal advice, employment, consumer, leisure and learning. Support is also provided on complex issues such as future living arrangements, care home fees, equity release and funeral arrangements
  • Health and well-being
    Activities include physical activity classes, movement to music sessions, walking groups, healthy eating workshops, reminiscence, mindfulness sessions and health promotion information
  • Creative Activities
    An opportunity to participate in a range of art and craft activities, creative writing, singing and music etc. to learn a new skill, reduce loneliness and improve emotional well-being
  • Digital inclusion
    We provide digital inclusion support in a fun and practical environment, helping people improve confidence and access to services. A range of digital equipment is available or you can bring along your own equipment to our gadget drop in sessions
  • Excursions
    We organise regular day trips to places of interest and various tourist attractions which are individually priced. Please get in touch for information on upcoming dates and destinations
  • Foot Care Service
    This service helps to reduce falls and improve independence and is delivered by fully trained staff and volunteers, all of whom are retired nurses and/or healthcare professionals. A number of our Community Outreach Hubs host regular clinics, as well as a regular drop in service
In addition to formal outreach services, we also attend public places such as shopping centres and supermarkets throughout the County Borough to promote our range of services and products.
